Chuck Jr. is survived by his son, Trevin Balzer; his parents, Chuck & Terri Balzer; his sister, Felisha Chentnik; brother-in-law, Greg Chentnik; niece and nephew, Holly & Hunter Chentnik. He also is survived by his Fiancé, Tiffany Kolomitz.
The Celebration of Life will be held in the Spring or Fall.
