Dr. Evangelista will be remembered for not only being an excellent physician but also a devoted and trusted friend, neighbor, brother, uncle, father, and grandfather. He was a loving and proud father to Nicole (Will) Brandt of Powell, OH and Nathan Evangelista of Phoenix, AZ. He is adored by his beloved grandsons, Mason and Michael Brandt.
